Claims Procedures
 
 
Please mail all claims and documentation to the following address to assure prompt claim service:  Central Processing Service, P.O. Box 6018, Ridgeland, MS  39158
 
If you have any problems obtaining any of the required documentation for a claim, please call Vickie Moseley at 1-800-333-5043 to discuss. We also ask that you MAIL all claims instead of faxing. Faxed copies of loan documents, certificates of insurance, and account histories are very difficult to read. 
 
 
To file a life claim, please submit the following:
  1. Properly completed claim form
  2. Copy of certificate of insurance
  3. Copy of loan contract
  4. Certified copy of death certificate
  5. Proof on net pay-off on loan
          Assurant form 1372-0200   -or-   American National form CG401


To file a NEW disability claim, please submit the following:
  1. Properly completed claim form. This form must be completed by the Creditor, the Insured, the Attending Physician and the Employer  
  2. Copy of certificate of insurance
  3. Copy of loan contract
          Assurant form  UL103-0703  -or-  American National form CG401


To file a CONTINUING disablility claim, please submit the following:
  1. Properly completed computer generated continuing claim form
  2. If your customer has lost their claim form and you must submit a continuing claim form, please write in the claim number on these forms
          Assurant form C2508  -or-    American National form CG401
 
  
To file an auto claim:
  1. Properly completed claim form with present location of the wrecked vehicle 
  2. A copy of the certificate of insurance
  3. A copy of the ledger card or account history
  4. Proof on net pay-offs as of the date of loss
  5. A police report and/or fire report
  6. Photographs of the damaged vehicle
  7. Two (2) written repair estimates
  8. Two (2) salvage bids
  9. A repossession affidavit indicating that you have possession of the vehicle in order to file a claim on behalf of the Insured
          Assurant form AL230  -or-  American National VSI Claim form
 
To file a fire claim:
  1. Properly completed claim form (link here )
  2. A copy of the certificate of insurance
  3. A copy of the ledger card
  4. Proof on net pay-off as of the date of loss
  5. A fire report and/or estimates of damages
  6. Copy of "Exhibit A" showing value of collateral in the event of a partial loss
